The Housing Support Co-ordinator

What is the role of the Housing Support Coordinator?

The Housing Support Coordinator can do many things to support you in your home.  Among other things they will:

 

  • Help you to settle into your new home by welcoming you to the scheme and showing you its facilities
  • Record contact details for your family, friends, doctor and anyone else who would need to be contacted in an emergency.  They will make sure this information is regularly updated.
  • Contact you daily Monday – Friday, to see if you need any assistance and to monitor your well-being.
  • Complete regular support plans so that, together, you can identify any assistance you require to help you live comfortably in your home.
  • Help you find out about activities and social events in the area and help you organise activities within the scheme.
  • Contact your doctor or family member if you are unwell, at all times respecting your wishes.
  • Collect emergency prescriptions if your doctor, pharmacist, family, friends or carers are unable to do so.
  • Provide practical help to complete forms and link with other agencies that may provide support.

Is there anything the Housing Support Coordinator does not do?

They are not allowed to:

  • administer any medication
  • provide any personal services (cleaning, cooking, shopping, bathing or assistance with dressing).
  • be an executor of your will
  • be responsible for your personal property
  • be involved in your personal finances

If you need help with these services you will need to make arrangements with family, friends, carers or other support agencies.

What is the Daily Visit?

The Co-ordinators will visit you daily, Monday – Friday, to see if you need any assistance and to monitor your well-being.

If your Coordinator is on holiday, sick, or training for up to two working days, staff will contact you from the Control Centre through the communications equipment. If absence continues after this time a suitable substitute will be appointed.



Do I have to stay in until the Housing Support Coordinator has visited?

You do not have to wait in for the Coordinator to visit. Please contact the Control Centre, using the easy to use communications equipment in your home, to inform them that you are going out.   The Control Centre will inform your Co-ordinator of your call, that all is well, and that a visit will not be necessary on that day.

What will happen if I don't inform the Control Centre that I am going out?

If your Coordinator or the Control Centre cannot make contact with you, they will assume that there is a problem, for example, you may have fallen or be ill.  In these circumstances, staff will telephone your nominated emergency contact to try to locate you.   If we are still unable to locate you, we will gain access to your accommodation to ensure that you are alright.

Staff will only gain access to your property without your permission in an emergency such as this.
